 A curriculum vitae for the chief official and key faculty of the international healthcare education program. Please underline or highlight all aspects of the document that represents how each individual has the education and experience to provide leadership in international healthcare education. Label document “Leadership Qualifications”

An organizational chart that shows the organizational structure and staff reporting relationships for the international healthcare education in relation to the parent institution, where applicable. Label document “Organizational Structure”

 If an institutional policy on scientific research exists, attach and label document “Scientific Research Policy”

 Documentation and attachment of the program elements and requirements specified under Accreditation Standards.

If Memorandums Of Understanding are entered into with service-learning healthcare facilities, attach copies of those and label “Memorandum Of Understanding”

Completion of the online Preliminary application form and payment of the application fee ($499).
